Hina, who is reluctant to join the Tokyo MER project, hopes that the ER car would have a fatal flaw in it, but all the equipment is just perfect.

Tokyo MER is formed by order from the Governor of Tokyo. The name of the group stands for Tokyo Mobile Emergency Room. The members are emergency care professionals and they are to go to sites of dangerous accidents and disasters by ER car. Their ER car is a large vehicle equipped with the latest medical devices and an operating area. There, Tokyo MER saves lives of the injured. Kitami Kota is a doctor for Tokyo MER and its leader. He holds a strong conviction that, regardless of the dangers, he will go to accident sites to save lives.

Set half a year after the drama, a collision between ships occurs on the Sumida River. The members of MER and the Tokyo Fire Department immediately respond. Will MER be able to rescue all the patients?

The individuals that comprise the Tokyo MER team rush to on-site disasters and accidents, performing life-saving treatment thanks to the OR equipped within their vehicle. Their one mission is simple: leave no one dead. But conflicts arise when an unprecedented event rocks Yokohama after a bomb explodes in Landmark Tower, leaving thousands fighting to escape. Although the chief doctor Kitami (Suzuki Ryohei) believes they should not wait and presses for a quick dispatch, the more level-headed Chief Kamoi (Watanabe Anne) of the elite Yokohama MER thinks otherwise — they need to wait until the area is safe to enter, or they won’t be able to save anyone.

Seventy floors up in Landmark Tower, 193 are left waiting for help. Explosions continue, fire draws near, and the number of wounded only rises, but the rescue helicopter has been rendered unable to approach. When it’s confirmed Chiaki (Naka Riisa) – now pregnant and married to Kitami – is among the 193, images of Kitami’s late sister flash through his mind… No one, he is determined, will die!

The narrative of this movie unfolds against the backdrop of the vast ocean stretching across Okinawa and Kagoshima, depicting a medical team that fearlessly fights to save patients through high-stakes rescue missions. Following TOKYO MER and YOKOHAMA MER, a newly formed Nankai (Southern Sea) MER team has been established.

In the first movie, the MER team took on a mission in Yokohama, followed by high-stakes operations across Okinawa and Kagoshima in the second film. Their next stage is Tokyo – the very birthplace of MER.
As the title suggests, the story unfolds in Japan’s capital, Tokyo, which has plunged into a critical crisis.
The film follows the MER team as they face the biggest – and final – battle in the entire series.
 
A massive earthquake strikes Tokyo with an eerie roar, instantly crippling the capital’s core functions.
