A major earthquake occurs immediately below Tokyo. This series depicts the members of the emergency room staff, confronting life head on as they give people hope and courage. This popular TV drama series depicts the members of an emergency room team continuing a daily battle to save the lives of people threatened with death. This third group of installments tackles a major theme for lifesaving operations, namely, emergency medical operations in case of a major disaster. The result is human drama on a grand scale. Dr. Issei Shindo, a member of the International Association of Physicians for Humanitarian Assistance, returns from abroad for a short stay in Japan. Dr. Kaede Kojima, who was formerly active as a research physician under Shindo, has now developed into an outstanding emergency room doctor. Meeting Shindo by chance, Kaede announces to him that she will give up her job in the emergency room. Shindo promises her that, before leaving Japan, he will visit her at the hospital and listen to her story. It is when Shindo is on his way by taxi to the hospital where Kaede is working that a major earthquake occurs directly below Tokyo.

The members of Emergency Room 24 must deal with chronic lack of manpower, lack of beds, & lack of doctors. Hospitals now often refuse to accept patients leaving ambulances to search for the next hospital in the vicinity. In the fourth season, a bride collapses during her wedding and is whisked away by paramedics. The ambulance then have trouble finding a hospital willing to accept the patient. The condition of the bride worsens, while the groom becomes combative to the paramedic team. The ambulance stops in the middle road and at that time, Dr. Kazuo Shindo steps out of the taxi to assist the ambulance

Kojima Kaede heads a team as the chief medical officer of the emergency and critical care centre at Minato University Hospital. However, the staff members of the centre are all skilled but idiosyncratic people with the emergency doctor Honjo Masaharu, heading the list. It is a defective “team” which is not able to work together. Kaede herself is a more competent emergency doctor than anybody else but is unsure of leading the team. Meanwhile, the hospital management is frustrated with the emergency and critical care centre for not having a single organ donor since the organ transplant act was amended. They decide to get the unusual emergency doctor Natsume Mamoru to satisfy both a high percentage of lives saved and number of organ donors. Under continual pressure from daily tasks, Kaede has no time for that. But on Natsume’s first day at work, the worst ever major incident in history breaks out in Tokyo.
