Synopsis
Out in the distant sea as part of Japan’s westernmost end lies the beautiful and isolated Shikina Island. Arriving after a six-hour ferry ride nineteen years ago was Goto Kensuke (Yoshioka Hidetaka), nicknamed Dr. Koto, who has since shouldered the responsibility of caring for the inhabitants as the island’s only physician, cementing himself as an irreplaceable family member for locals over years of building mutual trust. Now married to nurse Ayaka (Shibasaki Ko), they both anticipate welcoming the newest Shikina resident with the birth of their child only a couple months away.
Although Dr. Koto has been running his clinic alongside Ayaka, Wada (Kakei Toshio), newcomer Dr. Oda (Takahashi Kaito), and local Nurse Nishino (Ikuta Erika), the life and future of the island is not headed toward greener pastures. Like many of Japan’s regions, Shikina is facing a population crisis as locals age and residential influx stagnates. During talks of unifying healthcare amongst neighboring islands that are suffering from economic hardship, Dr. Koto is offered a position at a hospital — but this means leaving the place he’s called home for almost two decades. While this would be for the sake of the island’s future, he can’t bring himself to reply…
It’s during this time that a typhoon makes its approach toward Shikina—nothing the inhabitants aren’t used to, hunkering through multiple typhoons every year. But this one may be different when talk of damage beyond imagination comes up at the town hall…
One by one, emergent cases roll into the clinic, forcing Dr. Koto and his staff to cope with the limited medical system by repurposing into a field hospital. Once again, they are met with the kindness of their islander family and the value of human life. The sometimes-merciless nature, the new life that dwells over time, the things that are lost, the realities that stand in the way — the island encompasses everything and the people who live there.
